For your application form to be considered complete, you must do the following:
- Submit Application Form Part 1
- Click on the "Proceed to Checkout" button
- Pay the $150 application fee
- Submit Application Form Part 2
- Make sure you are using the latest version of your internet browser (e.g. Chrome, Firefox, Safari).
- Information you enter in each field is saved as you enter it.
- DO NOT use your browser's forward or back buttons as this may cause you to lose data. Use the "Continue" and "Previous" buttons at the bottom of the page to navigate through the form.
- Please review your information carefully before you submit. Once you submit your application, you cannot make changes.
